Consent and Agreement
I, Parent/Guardian of the participant hereby consent the name above to participate.
I understand that participation in the summer program involves various activities, including but not limited to academic workshops, and recreational activities.
I acknowledge that there are inherent risks associated with participating in the program activities, and I agree to take responsibility for own safety and well-being of participant/s during the program.
I authorize the program organizers to seek emergency medical treatment for the participant/s in the event of an injury or medical emergency, and I agree to provide accurate information regarding any allergies, medical conditions, or medications that may affect the participation of participant/s.
I understand that photographs, videos, or other media may be taken during the program for promotional and documentation purposes. I consent to the use of my likeness and/or image in such materials without compensation.
I agree to adhere to the rules, regulations, and guidelines set forth by the program organizers and to conduct myself in a respectful and responsible manner towards fellow participants, staff, and volunteers.
I understand that failure to comply with program rules or engage in behavior deemed inappropriate or disruptive may result in the dismissal of participant/s from the program.
I release and hold harmless to ASBT, its employees, agents, and volunteers from any liability for personal injury, property damage, or other loss arising from the participation in the summer program.
